For technical issues:
Web browser used and version number. Such as IE8 or Opers 9.64.
What page the problem occured on.
Steps leading up to the problem.
Is it duplicatible?

For spelling/grammer issues:
What page the issue is on.
General where on the page.
What the problem is.
